Incomplete requirements lead to errors in software development and also prevent
these errors from being detected during the testing phase. The resulting operational
failures may cause major economic losses or even casualties, and correction is far
more costly than if the defect had been caught earlier. We describe what is missing
in requirements, then why it is missing, and then explore remedial measures and
test strategies for verification of reliable software. Failure data analysis is
recommended to facilitate better elicitation of requirements.
